Alabama RHTP Statewide EMS Trauma and Stroke Initiative

Summary

Funds statewide EMS trauma and stroke coordination expansion to improve emergency routing, response capacity, and rural patient outcomes in Alabama. Supports practical, scalable strategies for initiation and expansion of hospital-referral services, protocol implementation, training, care coordination, and operational support.

Eligibility

Eligible applicants include EMS agencies or other qualified entities with experience coordinating multi-partner EMS and hospital networks capable of carrying out eligible Statewide EMS Trauma and Stroke Initiative projects. Applicants must have a valid Unique Entity Identifier (UEI), maintain an active SAM.gov registration, and demonstrate capacity for system development, protocol implementation, training, care coordination, and operational support. Applicants do not need to be physically located in a rural area, but must clearly demonstrate benefit to rural Alabama communities.

Requirements Checklist

• Applicants must submit all required documentation, including: Certificate of Existence from Alabama Secretary of State (issued within 30 days), current SAM.gov registration, signed IRS Form W-9, Alabama Vendor Disclosure Statement, written policies/procedures on conflicts of interest and internal controls, financial statements for last three years, audit reports (if applicable), project plan (ADECA template), grant budget workbook and narrative, documentation for matching funds/in-kind contributions, and any project-specific supporting materials required by ADECA.
