AHEAD Model Pre-Implementation Funding

Summary

the Rural Health Transformation Program. Eligible rural hospitals that have signed an LOI to participate in the AHEAD Model may apply for up to $750,000 to support migration to the new payment system, modernization of technology, workforce recruitment and retention, primary care outreach, and regional collaboration. There are 87 awards available. All funding awarded must be spent by September 30, 2027.

Eligibility

Rural hospitals in Georgia that have signed a Letter of Intent (LOI) to participate in the Achieving Healthcare Efficiency through Accountable Design (AHEAD) model pre-implementation planning.

Requirements Checklist

• Applicants must submit their application through the GREAT Health Application Portal within the specified deadline. Allowed uses include technology and infrastructure modernization, workforce recruitment and retention, and preparing for transition to the AHEAD payment model. Follow all instructions in the application portal.
