AFA - Mobile Medical Primary Care & Dental Care - FY2027

Summary

Announcement of Funding Availability for Health Right Free Clinics - Mobile Medical Primary Care and Dental Care Service. Up to six awards available, maximum funding $1,000,000, supporting mobile primary and dental care for underserved and rural communities across WV, especially Medically Underserved Areas and Health Professional Shortage Areas. Performance period July 1, 2026 through June 30, 2027. Funding prioritizes regional diversity, capacity for outcome tracking/reporting, and incentivizing healthy behaviors. The scope includes primary medical care, oral care, obstetrical care, mental health/substance use disorder services, hearing screening, physical/occupational therapy, pharmacy, diagnostic laboratory/x-ray procedures, and must complement (not duplicate) other funded activities. Applicants must propose only the amount reasonably needed and will enter a 12-month subrecipient grant agreement, with possible renewal.

Requirements Checklist

• Applicants must submit: Proposal Narrative; DH Detailed Line-Item Budget worksheet (Attachment B); Budget Narrative (Attachment C); Proof of Clinic Operation in West Virginia; Technical Assistance Documentation (if applicable). Budget must include a dedicated line item (recommended 10%) for technical assistance/evaluation. Applicants must comply with 2 CFR 200 Subparts B–E for cost principles and administrative requirements. Submission must be made via WV Oasis Vendor Self-Service Portal at www.wvoasis.gov
