The Rural Capital Project Care Gap Closure Grant Program (RCGC) provides capital funding for facility renovations, infrastructure projects, equipment acquisition, and service expansions that address healthcare gaps in rural Mississippi. Eligible applicants can receive up to $3 million per project to increase service capacity, improve healthcare access by reducing the travel burden, support rural hospitals and clinics, enhance operational efficiency, and strengthen healthcare outcomes in underserved rural areas. The RCGC is administered by the Mississippi State Department of Health as part of Mississippi's Rural Health Transformation Plan under a cooperative agreement with the Federal Rural Health Transformation Program. Program Year 1 will allocate up to $28.5 million, with individual awards anticipated for 20–30 projects. Projects must align with at least one program objective—capacity expansion, access improvement, hospital support, operational efficiency, or closing care gaps—and present a credible plan for achieving measurable results. Indirect and administrative costs may not exceed 5% of the award. The program excludes funding for telehealth, cybersecurity, EHRs, broadband infrastructure, new construction, major building expansion, lobbying, and provider payments. All funds must be obligated by October 30, 2026, and spent, or incurred, by July 31, 2027.
Eligible Providers include Mississippi-located hospitals, physician practices (including solo practitioners), rural health clinics, federally qualified health centers, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, substance use disorder facilities, dental care services, and licensed long-term care facilities located in or serving rural areas or populations. Consistent with CMS guidance, organizations do not need to be physically located in a rural area to be eligible, but must demonstrate meaningful benefit to rural communities and residents of Mississippi. 'Rural' is classified as counties with fewer than 50,000 residents, fewer than 500 people per square mile, or municipalities with populations under 15,000.
